Instructions

Contact our main office at 559-277-7577 for an RMA number. Our customer service team will assign an RMA number and send you a PDF of the form. Once you fill out the form in its entirety, please return and we’ll provide instructions.

Requests must be made within 30 calendar days of the date of shipment. Return of up to two separate products from the same order can be requested with this form.

All returns must include the approved RA form, goods in original packaging with RA # on box (or boxes), and a description of the problem. In case of shipping damage, please include a Bill of Lading.
